Record a self-reported heart-health check-in by text or voice, including symptoms, optional readings, medication concerns, care-team questions and callback preference.

Who it is for

Teams that collect check-ins from Members already in a heart health program.

What it does

  • The Heart health check-ins Form stores how you feel, symptom notes, reported blood pressure, pulse, weight and unit, medication concerns, care-team questions and callback preference. Unknown readings can be skipped. It keeps one current record per Member with the latest reported values.
  • The patient-facing Workflow asks focused questions and saves supported answers through the Form tools. Corrections update the same record and retain other answers.
  • The draft Journey reads Callback requested at enrollment and again after seven days. True adds the Callback requested label; false goes to the check-in email step. An unset callback preference adds Review callback status. Stop heart health ends the Journey.

What it does not do

It does not diagnose, score clinical risk, recommend treatment, schedule calls or notify a coordinator. Saving symptoms, readings or a question is not a clinical review. Use your clinical process for urgent symptoms. The Form is a latest-value record, not a separate dated record for every check-in.

Before you turn it on

Installing adds the Form, the patient-facing Workflow and a draft Journey. Automatic enrollment is off and email steps are inactive. Review the questions, clinical handoff process and Inbox, then test with synthetic Members before publishing the Journey. Confirm PHI authorization before using real health data.
